In collaboration with the Venice Family Clinic, the Ocean Park Community Center (OPCC) Project Healthcare for Empowerment to Access Respite, Treatment, and Housing (HEARTH) has used HHPF-CCP funds to establish a new medical/health care home for the homeless population in Santa Monica. Primary health care services, permanent supportive housing, and a new pilot respite bed program has assisted in the decline of outpatient and inpatient charges to local medical care centers.
